The hard limits are that banners must be between 400 and 800 pixels wide and 100 to 250 pixels high.  You should also tweak the JPEG quality down to minimize the file size; there are only a few situations where a banner should use up more than 75KB.  Opinion varies on the optimum dimensions - read through the banner thread for these.

Haven't used it myself, but people seem to give good reviews to Paint.NET.  I use the GIMP which is very capable but has a bit of a steep learning curve.  If (and only if) you are already familiar with Photoshop, GIMPshop may be useful as it is the GIMP with the interface altered to resemble Photoshop.

Square already answered #2. As for #1, it should be fairly evident that a long/wide image will work better (provided you are not cropping, turning, or otherwise sampling the graphic downward). F'rinstance, the shots at that page are mostly tall, and will not lend themselves well to banners unless you're just using a horizontal swatch of the image. The monkey on the couch would be a good example, though I am not specifically advocating its use (at least, not without a really inventive tagline/slogan).

I stick with 200x800. It fits nicely at the top of the screen and provides plenty of space to work with. Pale tends to be a little more catholic in the dimensions he uses.

I don't recall any specific info from any of your posts that would help a stalker.

By the standards of most of the people I know, I'm a little on the paranoid side; but of course I think I'm just realistic.  My epiphany came one idle afternoon when I got to thinking about my first-grade boyfriend and since I had nothing better to do, I Googled him.  I only had his nickname (from first grade!) and his surname.

It took me, literally, five seconds to find him.  I found out the name of the neighborhood where he lives (though not the specific address), the name and address of the company he works for and his position there, his wife's name, his daughter's name, age and birthday, and a recent photo.  I'm sure I could easily have found info like his wife's workplace and his daughter's school, if I'd been of a stalkery bent.

I didn't contact him; I was too freaked out by the Power of the Internets.  It was an important lesson.
